Objectives:
• Explore the coordination of care and collaboration of efforts for providers and parents of children with ASD/DD
• Discuss resources to further support children with special healthcare needs
Main Presentation:
• Fox Valley Children’s Mental Health Center and Collaboration of Care
Registrants will be able to attend three of six break-out sessions covering a variety of topics. Session choices will be:
• AAP Mental Health Toolkit
• AAP Autism Toolkit
• Care Notebook
• Navigation Guide
• Open Discussion- What works/What I am looking for
• Medical Home
A light continental breakfast will be provided at 8:15am during check-in.
